The aim of the project is to engage with and support the parents/carers and families of young people with special educational needs (SEND), who have a realistic change of progressing into employment and/or training.  To support parents/carers through the transition process.  Empowering parents and families to become informed users and partners in assessment and provision.  To work alongside Parent Partnership to identify families to support.

Empowering Families Adviser

I can support parents and families to help overcome the barriers to participation and achievement.  Help and support with transition planning, provide information on the options that may be available to their young person, how to access the options.  Help with issues around; Information, Advice and Guidance, Post school options, Person Centred Planning, Learning Difficulties Assessment (139A), new Education, Health and Care Plan.  Also, to contribute to the transition process by working closely with other professionals that may be involved.
